Position Summary

Assess bed availability.

Assess patients waiting and develop a plan for all Admissions and Transfers where appropriate. Communication of placement plan.

Identify Clinical and Departmental barriers and contact appropriate personnel.

Identification and documentation of system issues.

Responsibilities

Assess bed availability.
-Using a variety of methods to include: SMS, Wellsoft, CompuRecord, Patient Placement worksheet, Bed Status Report, Bed Tracking, and Daily Bed Briefing, Alpha pagers, communicate with Patient Care Coordinators, and Patient Placement staff, to determine capacity.
Assess patients waiting and develop a plan for all Admissions and Transfers where appropriate. Communication of placement plan.
-Using Wellsoft, and communication with Physicians, Charge Nurses, and Patient Placement staff, evaluate acuity and length of stay to determine placement priority.
-Document and distribute via verbal modes, fax, and email the plan to clarify expectation of patient movement.
-Ongoing evaluation, communication, and documentation of dynamic placement plan to Patient Care Coordinators, Patient Placement staff, and others as needed.
-Initiate Capacity Alert Level
Identify Clinical and Departmental barriers and contact appropriate personnel.
-Using any means or methods available including escalation to appropriate administrative level, identify clinical and departmental throughput barriers to expedite movement and discharges.
Identification and documentation of system issues.
-Identify trends both positive and negative and submit to Process Manager for review.
-Process Manager will investigate, analyze, and communicate recommendations for improvement.

Minimum Requirements

BSN required. MSN preferred.

NC RN license required.

Five years of clinical experience, with at least one year of management or leadership experience is required.

Evidence of progressive leadership qualities is required
