Position Summary

The Department of Care Management utilizes the Team Leader model to enhance the operational and clinical effectiveness of the department and to ensure seamless management of the day to day activities of staff engaged in discharge planning, care coordination activities, social work and capacity management.

Minimum Requirements

  • Masters of Social Work degree from a CSWE accredited school of social work is required with minimum two years case management experience
  • Or a BSN in nursing from an accredited college or university required with minimum 2 years case management experience is required
  • Certification in Case Management required within 2 years of hire is required
  • BLS CPR Certification within one year of hire
  • CPR training within one year of hire
  • 1 year to 3 years required in Case Management/discharge planning, preferred acute care case management
