The BH Case Manager is responsible for:
Implementation of utilization review processes such as application of admission/continued stay criteria approved by the medical staff at Vidant Medical Center (VMC), collaborating with other members of the interdisciplinary team to facilitate care at the most appropriate level and in the most effective, efficient manner, communicating with payer sources to assist with management of patients continuum of care and managing the data for the analysis of patient/program outcomes
Coordination of the discharge plan of the care of selected difficult cases through collaboration with the interdisciplinary team and focusing on timely, safe discharges and assuring placement of patients at the most appropriate level of care post discharge.
BSN or MSW, such as social worker, counselor, and/or marriage and family therapist, preferred.
Bachelors in social work, psychology or related field or ADN with 1 year UM experience required.
If Master's level therapist, prefer full license.
