The Care Management/Utilization Review Coordinator (CMURC) position will serve to support the discharge planning and utilization review (UR) processes by coordinating post discharge services and assisting with utilization review processes with oversight by the Nurse Care Managers (NCM), Utilization Review Specialists and/or the Social Work Care Managers (SWCM) within the Case Management and Utilization Management Departments. The CMURC may assist in collection of patient information, discharge care needs, regulatory interventions as well as work to facilitate a transition plan that promotes a safe and effective discharge and utilization of services. The CMURC will perform other activities related to the discharge planning and UR process as requested.
