Vidant Roanoke-Chowan Hospital is a 114-bed, not-for-profit hospital located in Ahoskie, NC. The 15-bed ER serves approximately 21,000 patients per year from the local community and surrounding towns. This modern facility provides a wide range of health services to its 39,000 residents in the four-county area.

  • Completes initial client/family assessments and problem lists.
  • Accurately identifies and prioritizes patient problems using critical thinking skills.
  • Collaborates with client, physicians, and other providers to develop a comprehensive plan of care.
  • Schedules home telehealth encounter sessions in collaboration with the health care team and informs patients of home telehealth encounter schedule.
  • Effectively communicates care plans and other client-related activities both verbally and in writing to appropriate personnel and agencies.
  • Utilizes clinical skills and assessments to work autonomously in outpatient, community settings, home telehealth, and to communicate with physicians and other providers re: alterations in the plan of care.
  • Demonstrates the ability to use teaching, learning, and counseling skills.Connects patient’s unit in the home and adequately instructs patients and/or caregiver on use and responsibilities.
  • Documents home telehealth encounters on appropriate visit forms and collects data as necessary.
  • Properly removes and cleans patient units when patients are discharged from home telehealth.
  • Maintains equipment logs and biomed checks.
  • Acts as a client advocate for the development of community resources.
  • Maintains a directory of available services by geographical area and updates others about new services.
  • Consults others to identify potential community resources for resolving client health, psychosocial, or financial problems.
  • Functions as liaison to external agencies and relays information to others, which may impact care and/or services of clients.
  • Demonstrates the knowledge and skills necessary to provide service appropriate to the age of the patient.
  • Develops and initiates cost saving strategies to achieve decreases in patient resource utilization.
  • Collaborates effectively with departmental/program staff to achieve goals and objectives.
  • Maintains productivity and optimum client caseload as negotiated with department director.
  • Ensures collection of date for outcomes and other studies as requested.
  • Provides information for maintenance of case management client database.
  • Supports efforts to implement disease management model.
  • Demonstrates effective interpersonal communication skills.
  • Facilitates productive meetings with interdisciplinary team members from across the system.
  • Develops strategic long-range plans in area of specialization.
  • Participates in financial planning for specialty care.
  • Advances clinical expertise through attendance at conferences, reading journals, etc.
  • Maintains a professional network among colleagues.
  • Participates in quality improvement (QI) activities.
  • Encourages clinical problem identification and resolution activities.
  • Role models use of QI principles and techniques.
  • Identifies, addresses, and reports quality issues identified through data sources.
  • Collaborates with regional hospitals
  • Identifies vulnerable high risk ED and inpatients.
  • Educates hospital team members and patients about available Network services.
  • Formulates effective post discharge care plan in conjunction with facility care managers.

 Minimum Requirements

  • Current RN licensure in the state of North Carolina or temporary North Carolina licensure or meet requirements to hold RN licensure in a compact state.
  • BSN in nursing from an accredited college or university required.
  • Evidence of at least three years of clinical competence in providing direct nursing services to both acutely and chronically ill patients in both inpatient and outpatient settings.
  • Evidence of progressive leadership experience either as a charge nurse or primary nurse essential.
  • Evidence of at least one year of previous experience as a nurse case manage or discharge planner with clients in the clinical specialty area desirable.
  • Evidence of a high level of skill in assessment, identification of problems, care planning, identification and development of community resources, and patient/family teaching required.
  • Evidence of participation in career development for professional growth through use of literature, research, and educational activities 

Preferred Requirements

  •  Masters degree in nursing from an accredited college or university preferred

  • 1 full-time position available
  • Pay range $22.60-$36.16/hr.  Actual rate depends on experience
