Our hospice team provides support for patients and the families of patients with a limited life expectancy. Using a team approach, we work to meet the special physical, emotional, social and spiritual needs of our patients and families.     

The Transition Care Coordinator is responsible for ensuring continuity and seamless transitions of patient care between settings. The Transition Care Coordinator is responsible for coordinating referrals to and from community based and ambulatory providers. The Transition Care Coordinator will act as a Care Coordination service resource for internal and external customers.


  • Referral Intake / Care Coordination: Receives and forwards new referrals to Care Coordination central intake in a timely manner to ensure timely admission and appropriate patient care based on assessed patient needs, physician orders, and available ambulatory resources. Ensures continuity and seamless transitions of patient care.

  • Collects information on referrals -completely, timely, and accurately. Acts as a resource for referral sources. 

Minimum Requirements

  • Currently licensed as Registered Nurse by the N.C. Board of Nursing is required.

  • Current BLS (Basic Life Support) required.

  • Minimum of two years clinical experience, preferably in home care is required.
