Position Summary

  • Accountable for developing, facilitating services, and partnering with community resources to address the needs of an assigned population.
  • Uses independent, professional judgement to provide case management services to assigned population.
  • Coordinates and develops educational opportunities related to assigned population for a variety of stakeholders (ie the healthcare system, school system, community, employers, health departments patients and their families)
  • Provides leadership in population based health improvement.
  • Maintains and advances knowledge and skills in areas of specialization.
  • Promotes and participates in quality improvement (QI) activities and clinical research.

Minimum Requirements

  • Master's degree in Social Work required. In some designated areas of the hospital a licience may be required.
  • 2-3 years related experience required.
  • Current Basic Life Support (BLS) Certification

Other Information

  • 1 full time vacancy
  • May work 1- 2 days per week at Vidant Roanoke Chowan and will cover Bertie, Gares, Perquimans, Chowan, Currituck, Camden and Dare counties.
