  • Program Development - utilizes principles of program coordination to effectively meet patient needs. Coordinates development of social service case management program policies, procedures, goals and objectives. Coordinates evaluation of social service case management programs, addresses barriers, and recommends changes. Precepts Social Worker Assistants as assigned.
  • Direct Patient Care - provides social service case management to ambulatory care coordination patients. Completes social service assessment for all referred/assigned patients. Participates in the development of the Plan of Care and provides information regarding patient needs for coaching, home and community services and referrals. Provides patient and family training, counseling, education, and linkage to available resources.  Observes, records, and reports to the physician, Clinical Case Manager, and other appropriate health care team individuals any reaction to treatment or changes in the patient's condition. Communicates effectively with other health professionals to enhance the total patient care.
  • Demonstrates Service Excellence.
  • Participates in Continuous Quality Improvement activities. Follows Patient Safety guidelines, organizational policies and procedures, professional standards of care, best practices, and regulatory requirements related to patient care quality. Participates actively in Patient Safety and Quality Improvement activities. Consistently "Calls the Question" regarding patient safety or quality issues. Consistently uses good judgement and thinks critically in order to make correct decisions.

Minimum Requirements

  • 4 year college degree plus a Master's Degree in Social Work is required.

  • Must have at least 2 years experience in the practice of social work in an acute care facility, a skilled nursing facility, or a Department of Social Services; 1 year experience in home health or ambulatory setting preferred.

Other Information

  • Some travel to Vidant Medical Group Practices required.
