Position Summary

Administers nursing care with appropriate knowledge and skills to critical medical and telemetry patients. Supports the patient and the family with the dying process. Performs admission assessments and subsequent assessments collecting data reflective of the patient's status, and interprets collected information to identify each patient's nursing care requirements relative to his/her age-specific needs. Provides care as needed and described by Health System policies and procedures. Completes all required documentation. Addresses patients' learning needs and ability. Works to promote general health by educating patients and family members on warning signs and symptoms of specific disease processes. Develops nursing care plans, executes them, and discusses plan with patient and family members. Enters orders in the computer and participates in daily operation of 2 East. Attends required inservices, yearly education, and departmental meetings. Participates in the collection of Performance Improvement data. Adheres to Health System and departmental policies and procedures. Practices effective communication with peers, physicians, management, and all other stakeholders.

Responsibilities

  • Assesses and diagnoses patient and family needs to provide quality care to assigned patients.
  • Performs admission assessment in accordance with specific unit standards (including, but not limited to, physical and social assessments).
  • Identifies and documents nursing diagnosis on patient's plan of care in accordance with specific unit standards with LPN oversight per policy and procedure.
  • Identifies and documents patient/family/significant other needs upon admission.
  • Verifies and documents informed Consent per policy and procedure (surgical, conscious sedation, blood administration).
  • Identifies discharge planning needs of patient/family/significant other on admission.
  • Identifies and provides appropriate education and support to patients and family experiencing the dying process.
  • Develops, discusses, and communicates a realistic problem list (plan of care) for each patient, in collaboration with each patient/family/significant other, in order to address all identified needs.
  • Plan of care will include nursing diagnosis statement, interventions, 24-hour outcomes, and discharge outcomes for each identified problem. This also includes updating plans of care for those patients in restraints, whether medical or behavioral.
  • Develops patient/family/significant other teaching and discharge plan as per unit standard.
  • Provides patient teaching on a continuous basis and documents using the integrated teaching tool.
  • Demonstrates knowledge of Health System policies and procedures for administering, transcribing and recording medications.
  • Demonstrates knowledge of commonly used medications: action, dose and side effects.

Minimum Requirements

  • Graduate of an accredited school of nursing required.
  • BLS current certification.
