Position Summary

The Transfer Center Registered Nurse is responsible for the coordination of patient transfers to VMC, to an appropriate Vidant Health Community Hospital or to a Non-Vidant Health Hospital. The Transfer Center RN collaborates with the Patient Placement Facilitator, Patient Placement Coordinators and the Communications Center Specialist, and acts as a clinical resource and facilitator for all interfacility transfers. The Transfer Center RN utilizes computer technology to facilitate multi-faceted communications and applies nursing knowledge derived from education and experience as foundation for transfer facilitation, communication, and documentation.

In addition to the basic responsibilities per RN job description, the Transfer Center RN facilitates all regional referrals, the Transfer Center RN may facilitate transfers, admissions, bed requests from inpatient, outpatient, clinics, pre-admits, and for cardiovascular and medical/surgical services.

Minimum Requirements

  • ADN required
  • North Carolina RN license required.
  • At least five years of experience in a patient care environment utilizing clinical and assessment skills.
  • Leadership experience required, i.e., charge nurse.
  • Charge nurse experience required.
  • EPIC user

Preferred Requirements

  • BSN preferred
  • American Heart Association BCLS & ACLS certifications preferred
  • Knowledge of admission, patient placement, transfer, and discharge processes preferred
  • Teletracking user experience is a plus
  • Utilization management experience a plus
