Position Summary: Program Coordinator – Education & Community Outreach (Advanced Care Planning) 

Overview: With primary emphasis on Community Outreach for Advanced Care Planning,  the Coordinator is responsible to establish partnerships and effectively collaborate with Vidant and community based individuals, groups, and agencies. Through this collaboration the Coordinator will lead the development, coordination and implementation of outreach initiatives to educate the public and providers to promote communication and completion of Advanced Directives. Success will be measured by the percentage of adults with an Advanced Directive on record in the Electronic Health Record.

  • Staff & Provider Education: Coordinate end of life and palliative focused education, competencies and tools for Vidant staff, providers, volunteers and students.
  • Community Outreach: Coordinate community and provider outreach initiatives, develop, promote and implement outreach, digital and marketing strategies.
  • Collaborate with and establish partnerships with Vidant and community based individuals, groups, and agencies to conduct EOL events, educational offerings and advanced directives clinics across the region.
  • Community Advocacy: Represent Vidant on professional and community collaboratives to advance palliative and end of life care. Establish and maintain collaborative relationships with a variety of key community stakeholders. Recruits volunteers to support Vidant End of Life services and programs.
  • Support Hospice & Palliative Care service line and Vidant End of Life initiatives Collect and utilize appropriate data to evaluate services, participate in standardization of protocols and practices across settings, participate in audits, quality and process  improvement activities. Other duties as assigned.

Minimum Position Requirements

  • College degree plus other schooling is required. 
  • Master's prepared in Nursing, Social Work, Health Education or related degree is required.
  • Additional 2 years experience in advanced care planning with Bachelors degree may substitute for required Masters degree. 
  • Minimum of 3 years employment experience in palliative, hospice or related end of life care required.
  • Certification in Hospice and Palliative Care specialty preferred. Facilitator certification in required Advanced Care Planning curriculum (currently Respecting Choices) required or may obtain within 3 months of employment. 
  • Valid NC driver's license is required. 
  • Experience as an ENS, CNS, and/or Program Coordination preferred.

Other information

  • Position is salaried.   Offer would be based on years of related experience.
  • Typical work hours are Monday - Friday, 8:00 am - 5:00 pm.  Flexibility may be needed to include evenings and some weekends based on the schedule of community events.
  • Travel using a personal car will be involved on a frequent / daily basis across the Vidant Health 29 county service area.  Travel expense support would be offered.
