Our goal is to enhance services that are available locally and to eliminate barriers involving time, distance and lack of awareness that sometimes prevent patients from receiving the care they need.

The Specialist is responsible to coordinate and conduct Community and Provider Outreach and engage in Advocacy initiatives to promote continuity of compassionate and appropriate care across all healthcare settings and to establish and maintain collaborative relationships with key community stakeholders. The primary focus of this position is Advance Care Planning and End of Life Care, but may also support promotion of other areas of Vidant HealthAccess/Care Coordination.

Responsibilities

  • Community Outreach: Coordinate community and provider outreach initiatives to promote continuity of compassionate and appropriate care across all healthcare settings. Develop, promote and implement educational and promotional events, materials and strategies to drive an awareness and utilization of services. Collaborate with and establish partnerships with Vidant and community based individuals, groups, and agencies to conduct events and educational offerings across the region. Manage requests for workshops, etc. in a timely and efficient manner. Coordinate and/or participate in public relations, marketing and community events such as health expos/ health fairs, church and community group presentations.
  • Community Advocacy: Represent Vidant on professional and community collaboratives. Establish and maintain collaborative relationships with a variety of key community stakeholders including but not limited to coalitions, other health care agencies, community/civic organizations and the faith based community. Acts as a VH liaison and promotes VH programs and services. Recruits volunteers to support Vidant services and programs.
  • Support HealthAccess service lines and Vidant strategic initiatives: Coordinate and instruct staff, provider and volunteer training for area of focus. Collect report and utilize appropriate data to evaluate services, participate in standardization of protocols and practices across settings. Participate in quality and process improvement activities.

Minimum Requirements

  • Bachelor's degree in Nursing, Social Work, Health Education or related field.
  • Minimum of 3 years experience in health based community outreach and/or education required.
  • 2 additional years experience in advanced care planning, palliative, hospice or related end of life care with Associates Nursing degree may substitute for required Bachelor's degree.
  •  Facilitator certification in required curriculum (currently Respecting Choices) required or may obtain within 3 months of employment.

Other Information

  • Valid NC driver's license and required insurance limits; frequent travel throughout the Vidant service area.
  • 1 Full Time Position Available, hours are Monday - Friday, 8:00am - 5:00pm with some nights/weekends required
  • Position will close on Monday, February 2nd, 2015
