Responsibilities

1.?? Direct Patient Care in collaboration with Collaborating Physician(s). Assists with admission work-ups and/or consults to facilitate care. Initiates, monitors, alters and orders treatments including but not limited to device placement, physical and occupational therapy, home health referral, and discharge planning.

Re-evaluates the patient throughout the hospitalization and modifies plan of care as needed in collaboration with clinical staff. Demonstrates the knowledge and skills necessary to provide service appropriate to the age and development of the patient. Perform diagnostic and therapeutic procedures as indicated for diagnosis and management of problems.Completes wound assessments, initiates basic skin care orders.

Order laboratory, radiological and diagnostic studies that will assist with the medical management; explain the necessity, preparation, nature and anticipates effects of treatments, and diagnostic and therapeutic procedures to the patient. Follows clinical practice protocols and guidelines agreed upon by supervising physician as stated by Collaborative Practice Agreement (CPA)
2.?? Documents physical findings and plan of care in the progress notes. Documents results of studies and plan of care based on study results in progress notes.

Chart completion with attention to coding. Documents legibly using hospital approved abbreviations.

Completes all elements of medical record, signs and authenticates orders as stated in the VMC bylaws.

Utilizes computer systems appropriately to collect, enter or view information.
3.?? Patient/family communication and coordination of the plan of care with patients and families.

Facilitates interdisciplinary plan of care through coordination with physicians, nurses, and allied health practitioners, including the use of care paths and protocols.

Participates in benchmarking and quality improvement activities. Participates in Advanced Practice Council and departmental meetings.

Contributes to clinical research. Incorporates current research into practice. Participates in research projects and data collection as appropriate.

Facilitates and completes orders and discharge summaries for facility placement.?? Writes discharge orders to expedite early morning discharges.

Facilitates discharge process to expedite throughput process. Completes discharge documentation to include summary of instructions and follow-up information provided to patient and family.
4.?? Fulfills mandatory educational requirements annually, which include but not limited to?? JCAHO and other institutionally required education. BLS and ACLS or appropriate certification related to specialty.

Annual Occupational Health Screening. Fulfills annual education as required by governing board(s). Demonstrates Service Excellence behaviors.

Participates in professional practice activities, conducts research and disseminates findings through publication and/or professional presentations.

Demonstrates effective interpersonal communication skills. Demonstrates effective written communication skills. Work effectively with others in a team approach.

Serves as a clinical resource.?? Participates in hospital committees as appropriate

Minimum Requirements

Board of Nursing approved Nurse Practitioner Program or Physician Assistant program.

NC RN licensure & NC Nurse Practitioner or as a Physician Assistant; National Board Certification as an NP/PA required.

More than 2 years experience in a health care setting.

May substitute NP/PA with no previous experience as NP/PA if they have clinical experience in an acute care setting with direct patient care for a minimal of 2 years.
Basic Life Support (BLS) Certification
