Position Summary

As a member of the Patient Care Services Leadership Team, the Case Management Manager is the primary provider of department based management and supervisory support to facilitate the safe, cost-effective, and high quality delivery of patient care.  The Case Management Manager combines critical thinking, clinical reasoning, management, and leadership skills to provide 24-hour accountability for case management services for the hospital.   Position will be responsible for data collection and management, fiscal health and adherence to departmental, hospital, and system policies and procedures.  The Case Management Manager serves as leader, coach, mentor, consultant, innovator, and change agent in the pursuit of exemplary coordination of care and care transistion. Must possess knowledge of DRGs,  Interqual Criteria, Medicare Medical Necessity, ICD 9 and ICD 10 Coding.    Position will provide leadership consistent with hospital and system goals and outcomes targets.  The Case Management Manager behaves in a manner consistent with the professional practice standards, state Nurse Practice Act, and Code of Ethics adopted by the nursing profession.
