We know that patients and families want to be in caring hands. Our Home Health services allow patients to recover at home in a more comfortable environment.


Position Summary

The Manager, Patient Care is responsible for the supervision and coordination of daily operational functions for Home Health and Hospice (inpatient and/or home based sites) clinical and support services. This responsibility includes oversight of care, delivery and services provided by office sites and clinical support services at all home based sites and/or inpatient hospice. Responsibility includes applicable programs of home health, hospice, social work, chaplain, bereavement and CAP. Maintains 24 hour accountability for assigned areas of responsibility. Twenty-four hour accountability for program management and planning for assigned agency sites, services, and associated staff.

Minimum Requirements

  • Bachelor Degree required.
  • A Licensed Physician, Physical Therapist, Speech Language Pathologist, Occupational Therapist, Audiologist, Social Worker or Registered Nurse is required.
  • American Heart Association Basic Life Support Certification required.


Preferred Requirements

  • Healthcare leadership experience strongly preferred
  • Home Health experience strongly preferred
