The Inpatient Social Work Case Manager is responsible for coordination of discharge planning, reassess and adjusts plan of care accordingly and utilizes the nursing process to plan, implement, and evaluate the discharge needs for patients in assigned caseload.

Responsibilities  

  • Inpatient Social Work Case Managers are accountable for the daily management of all complex discharge planning and social work functions for assigned patient care area using social work theory and clinical practice skills appropriate to the age of the patient incorporating the patient/family's social and emotional state.
  • Participates in quality improvement (QI) activities.
  • Advances knowledge and skills in case management.
  • Collaborates effectively with departmental/program staff to achieve goals and objectives.
  • Primary responsibilities include:
    • Proactive development, implementation and ongoing revision of an interdisciplinary post hospital plan of care. Negotiation and collaboration with the interdisciplinary team, community resources and payers to facilitate timely and cost effective discharge plans that enhance appropriate health and social services in the continuum of care are imperative.

Minimum Requirements

  • Four year college degree - BSW with a minimum of 2 years experience. MSW preferred.
  • CPR/BLS Certification recommended.
  • Three to five years related experience in acute discharge planning, home health, case management, long term care or related community agency

  • Full-time 8:30am to 5:00pm Weekdays, includes call and holiday
