  • The Inpatient Social Work Case Manager coordinates the delivery of discharge planning and social work services to all inpatient units of the hospital and designated outpatient treatment units. This role requires interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric.
  • The Inpatient Social Work Case Manager will provide direct intervention with patients and families and jointly with the health care team, will provide assessment, problem solving, advocacy, community referrals and crisis intervention services.
  • Inpatient Social Work Case Managers are accountable for the daily management of all complex discharge planning and social work functions for assigned patient care area using social work theory and clinical practice skills appropriate to the age of the patient incorporating the patient/family's social and emotional state.
  • Participates in quality improvement (QI) activities.
  • Advances knowledge and skills in case management.
  • Collaborates effectively with departmental/program staff to achieve goals and objectives.
  • Primary responsibilities include: Proactive development, implementation and ongoing revision of an interdisciplinary post hospital plan of care. Negotiation and collaboration with the interdisciplinary team, discharge plans that enhance appropriate health and social services in the continuum of care are imperative.

Minimum Requirements

  • BSW with a minimum of 2 years related experience. MSW preferred.
  • CPR/BLS Certification recommended.
  • MSW can substitue for the BSW with 2 years related experience.
  • Social Work experience in Women's & Children's area highly recommended.

Other Information

  • Full-time 8:00am to 5:00pm (call, holiday and weekend rotation required)
