About Vidant Medical Center
Vidant Medical Center is a fully accredited, 900+ bed regional referral facility encompassing a Level-1 Trauma Center and extensive inpatient and outpatient services including intensive and intermediate care, pediatrics, bariatrics, obstetrics and gynecology, surgery, pain management and psychiatry.
The Inpatient Nurse Case Manager is responsible for coordination of the discharge plan through collaboration with the physician, the interdisciplinary team, and the patient and family. The Inpatient Nurse Case Manager facilitates the discharge plan with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Inpatient Nurse Case Manager helps drive change by identifying areas where performance improvement is needed (e.g. day to day workflow, education, process improvements and patient satisfaction).
The Case Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay, promote efficient utilization of resources and ensure a well organized and safe discharge plan is established for every patient.
BSN or applicants currently enrolled in BSN with graduation date scheduled.
Current RN licensure in the State of NC.
BSN in nursing from an accredited college or university preferred with 2 years clinical experience, MSN preferred.
Clinical experience in critical care areas preferred. Evidence of leadership experience, e.g. charge nurse or primary nurse. Evidence of at least one year of previous experience with case management, discharge planning or utilization management preferred.
Evidence of a high level of skill in critical thinking, assessment, identification of problems, care planning, identification and development of community resources, and patient/family teaching required.
Evidence of participation in career development for professional growth through use of literature, research and educational activities
CPR/BLS Certification: REQUIRED
Full-time (M-F 7a-3:30p), other hours as needed.
