Position Summary

  • The Inpatient Nurse Case Manager is responsible for coordination of the discharge plan through collaboration with the physician, the interdisciplinary team,the patient and family.
  • The Inpatient Nurse Case Manager facilitates the discharge plan with the underlying objective of enhancing the quality of patient care, optimizing clinical outcomes and patient satisfaction while managing the cost of care. In addition, the Inpatient Nurse Case Manager helps drive change by identifying areas where performance improvement is needed (e.g., day to day workflow, education, process improvements and patient satisfaction).
  • The Inpatient Nurse Case Manager is accountable for a designated patient caseload and plans effectively in order to meet patient needs, manage the length of stay, promote efficient utilization of resources and ensure a well organized and safe discharge plan is established for every patient.

Minimum Requirements

  • BSN in nursing from an accredited college or university required with 2 years clinical experience.
  • RN licensure in the State of NC or hold a Temporary License as a Registered Nurse in NC
  • Evidence of a high level of skill in critical thinking, assessment, identification of problems, care planning, utilization of community resources, and patient/family teaching experience required.
  • CPR/BLS Certification that is AHA certified
  • Basic Life Support (BLS) Certification
