 Responsibilities

  •  The Emergency Department Social Work Case Manager coordinates the delivery of discharge planning and social work services to the trauma and emergency patient population.
  • The Emergency Department Social Work Case manager will provide direct intervention with patients and families and jointly with the health care team to provide assessment, problem solving, advocacy, community referrals and crisis intervention services.
  • The Emergency Department Social Work Case Manager serves as a resource to all inpatient units of the hospital as well as designated outpatient treatment units.
  • The Social Work Case Manager will provide interaction with a variety of patient populations including, but not limited to, neonatal, pediatric, adolescent, adult and geriatric as well as direct intervention with patients and families.
  • Primary responsibilities include:
    • Proactive development, implementation and ongoing revision of an interdisciplinary post hospital plan of care.
    • Negotiation and collaboration with the interdisciplinary team, community resources and payers to facilitate timely and cost effective discharge plans that enhance appropriate health and social services in the continuum of care.
    • Accountable for using social work theory and clinical practice skills appropriate to the age of the patient incorporating the patient/family's social and emotional state.
    • Participates in quality improvement (QI) activities.
    • Advances knowledge and skills in case management.
    •  Collaborates effectively with departmental/program staff to achieve goals and objectives..

Minimum Requirements

  • College Degree (4 yrs) - BSW with a minimum of 2 years experience.
  • MSW preferred
  • CPR/BLS Certification recommended.
  • Three to five years of related experience in acute discharge planning, home health, case management, long-term care or related community agency preferred

Other Information

  • Full-time 3:00pm to 1:30am (4 days/week), includes some holidays and weekends as needed.
