Position Summary

This position is responsible for partnering with local leaders in eastern North Carolina communities to promote health. The Community Health Improvement Coordinator works with a comprehensive group of leaders and elected officials from communities, various agencies and faith-based organizations to promote environmental and policy change to promote health. In addition, this position is responsible for implementing Vidant Health strategies in the region to educate, inform, and support the consumer in developing a healthy lifestyle.

Responsibilities

Commnunity Health Improvement Coordinator

  1. Leads community health improvement initiatives throughout Vidant Health's 29 county service area.

Serves in a leadership capacity on local and regional healthy living task forces focused on improving health through policy and environmental change.

Serves in a leadership capacity on local and district Community Transformation Grant task forces which implement national best practices throughout eastern NC.   

Partners with local agencies (including schools, parks and recreation, senior programs, and after school programs) to implement programming based on needs assessments. 

Partners with faith-based organizations to support leaders and lay health advocates with programming and needs assessments, as well as implementation plans.

Meets with local leaders, elected officials and policy makers to advocate for environmental and policy change throughout eastern NC to promote health.

 

  1. Advises and supports communities in the development, implementation and evaluation of health improvement strategies using communication and change theories.  

Provides leadership to community leaders, elected officials and policy makers in the assessment and evaluation of community health needs.

Serves as expert advisor and catalyst for implementing healthy communities strategies in eastern NC, based on national and state best practices.

Utilizes multiple data sources to evaluate and interpret the impact of community health improvement strategies on the overall health of the community. 

Reports evaluation of strategies and initiatives to community leaders, Senior Administrator of Community Health Improvement, Regional Hospital Presidents and Chief Exec Officer of External Affairs.

 

  1. Responsible for implementation of Vidant Health's Community Health Improvement strategic plan.

Identifies community health priorities for the Vidant Health service area in conjunction with the Senior Administrator.

Utilizes best practices to develop and implement Vidant Health's strategic initiatives for healthy communities.

Leads specific health initiatives for health system.

Markets Vidant Health programs and services in conjunction with Marketing and Public Relations.

Develops annual report to communicate Vidant Health's commitment to community health improvement to citizens.

 

  1. Represents Vidant Health on local and statewide committees, organizations, and agencies.

Prepares and presents information to various groups and boards as requested.

Participates in local and statewide committees and task forces.

Represents Vidant Health to internal and external organizations.

Establishes and maintains rapport and professional relationships with local leaders and elected officials, as well as professional colleagues.

Minimum Requirements

  • College Degree (4 yrs) in Nursing, Exercise Physiology, Family & Community Services.

  • Master's Degree preferred

  • Five years or more experience in a Community health leadership position, population based health program coordinator or supervisor; experience collaborating with multiple agencies; other supervisory, management or administrative roles.
