Vidant Edgecombe Hospital, formerly Heritage Hospital, is a full-service, 117-bed acute care facility where residents of Tarboro, Edgecombe county and surrounding communities receive a wide range of health services close to home.  Our partnership with Vidant Health enhances our wide range of services and improves the quality of care our patients receive. More than 20 specialties are represented by Vidant Edgecombe Hospital's medical staff, assuring the people of Edgecombe county that almost all their health needs can be met right here at home. In addition to acute care, our services include rehabilitation, oncology and outpatient clinics.

Responsibilities

  • This position reports directly to the Project Coordinator of the Community Care Plan of Eastern Carolina and is registered nurse who is responsible for working with the Carolina Access - Medicaid population.  The Clinical Case Manager collaborates with the client, and other health care providers to facilitate client access to those health and social services which enhance the client’s health and well being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible.  The Clinical Case Manager projects a positive professional image in both dress and manner.

Minimum Requirements

  • A Registered Nurse in the state of NC, with a minimum of three years nursing experience in an acute care hospital setting.   

  • Qualified candidates would be comfortable performing patient home visits as part of their daily routine.

Preferred Requirements

  • Previous discharge planning and community health background is preferred.

Other Information

  • This position will be based out of Wilson, NC.  The person will travel to perform home visits in District 5 (Nash, Edgecombe and Wilson counties).
