Position Summary

Provides coaching and disease management services to identified populations. Facilitates comprehensive care planning and oversees data management to support process improvement. Supports evidence based practice and medical home/care coordination principles.

Minimum Requirements

  • Licensed as a Registered Nurse in North Carolina.
  • Bachelor's Degree in Nursing preferred
  • Demonstrates competencies in nursing practice, with a focus on quality.
  • Basic computer skills with excellent verbal and written communication skills.
  • Minimum of  two years ambulatory experience. 
  • Education may be substituted with ADN degree and minimum 4 years ambulatory experience.
  • BLS Required - American Heart Association Current Basic Life Support (BLS) Certification
