Position Summary

This position reports directly to the Project Coordinator of the Community Care Plan of Eastern Carolina and is registered nurse who is responsible for working with the Carolina Access - Medicaid population.  The Clinical Case Manager collaborates with the client, and other health care providers to facilitate client access to those health and social services which enhance the clients health and well being and his/her ability to adhere to the prescribed medical treatment regimen in the most efficient and cost effective manner possible.  The Clinical Case Manager projects a positive professional image in both dress and manner.

Minimum Requirements

  • A Registered Nurse licensed in North Carolina or related compact state
  • Minimum of five years nursing experience in an acute care hospital setting
  • Previous discharge planning and community health background is preferred
