Position Summary

Provides coaching and disease management services to identified populations. Facilitates comprehensive care planning and oversees data management to support process improvement. Supports evidence based practice and medical home/care coordination principles.

Minimum Requirements

  • Bachelor's Degree in Nursing preferred
  • Demonstrates competencies in nursing practice, with a focus on quality.
  • Basic computer skills with excellent verbal and written communication skills.
  • Licensed as a Registered Nurse in North Carolina.
  • Minimum of  two years ambulatory experience. 
  • Education may be substitued with an ADN degree and a minimum of 4 years ambulatory experience.
  • BLS Required
  • Basic Life Support (BLS) Certification

Preferred Requirements

  • Experience in case management, disease management, or health coaching is preferred.

Other Information

  • Hours- Monday through Friday, 8A-5P
  • Position will perform outreach to patients seen in Vidant Emergency departments.
  • Goal is to improve patient experience, reduce ED utilization, and prevent duplication of outreach.
