This position is part of Health Access which includes Home Health, Telehealth, Hospice, Inpatient Hospice, Palliative Care, Transitional Care and Wellness Centers.

Position Summary

The Central Intake Liaison LPN is responsible for ensuring continuity between the referral sources, patients, and clinical providers in order to maximize patient satisfaction, patient safety, comfort, and well-being. The Central Intake Liaison LPN is responsible for coordinating referrals to ensure timely admission and appropriate patient care based on patient needs and / or physician orders. This person will act as a Care Coordination service resource for internal and external customers, including patients and staff.


  • Referral Intake/Care Coordination: Receives and processes new referrals in a timely manner to ensure admission and
  • appropriate patient care based on patient needs, physician orders, and available ambulatory resources.
  • Clinical Skills and Knowledge: Collects information on referrals complete, timely, and accurately. Acts as a resource
  • for referral sources.
  • Demonstrates Service Excellence: Communicates appropriately with staff and customers.

Minimum Position Requirements

  • High School plus less than 1 year of formal training or education.
  • Currently licensed as a Licensed Practical Nurse by the N.C. Board of Nursing required.
  • More than 2 years progressive clinical experience with minimum of two years experience in home care preferred.
  • Must maintain a valid North Carolina driver's license or be licensed in the state you reside and maintain the Vidant Health required minimum insurance limits.
  • Must maintain adequate/dependable transportation in order to have occasional travel

Other Information

  • Position is hourly.  Minimum is $14.60.    Offers would be made based on years of related employment.
  • Position will be based in Greenville, NC but travel to other locations in the region may be involved when needed.
