Vidant Medical Center is a 900+ bed Level-1 Trauma Center, regional referral hospital and is the flagship hospital for Vidant Health. We serve as the teaching hospital for the Brody School of Medicine at East Carolina University.  Responsibilities 

  • Works collaboratively with the NCM and SW using a "customer service" framework to facilitate timely discharges and support a smooth transition to the next level of care for Vidant Medical Center (VMC) patients. Interacts with patients and families to facilitate discharge needs.
  • Coordinates the communication and referrals to post discharge providers including home health, home and inpatient hospice, Long Term Acute Care facilities, Skilled Nursing facilities, Assisted Living Facilities, transportation and any other type of post discharge provider of services required. Serves as a contact for external providers regarding post discharge referrals. Gathers all pertinent information required by provider type and sends to providers as requested by the Uses the Release of Information system and/or other identified system (fax, email, US mail, etc.) to electronically send the pertinent information for each referral type. Conducts follow-up phone calls with providers, as needed, to confirm referral receipt and obtain disposition (approval or rejection). Provides notice to external provides who received referrals of provider selection. Updates NCM/SW to advised of status and outcomes timely and expeditiously. Coordinates the discharge date and time with the VMC team and post discharge providers. Conducts research to identify providers for specific services as needed and requested by the NCM and SW. Schedules follow-up appointments, as requested, to support post discharge follow care including, but not limited to, physician office visits, wound care clinic visits, etc. as time allows.
  • Demonstrates and maintains working knowledge of payer benefits (Medicare, Medicaid, Commercial payers) and payer-specific contracted providers to ensure compliance with state, federal and commercial payer requirements. Works collaboratively with the VMC business office and financial counselors to confirm payer and benefit coverage. Researches Medicare days used/available. Develops relationships with Commercial insurance companies to determine benefits available under the patient's plan as well as contracted providers.
  • Maintains daily activity work list to track all referral activities including active and discharged cases to produce productivity reports. Copies/prints medical records for disability and Medicaid as time allows and as requested.
  • Participates to ensure compliance with State and Federal regulatory requirements including delivery of the Important Message from Medicare.
  • Other duties as assigned.

Minimum Requirements

  • High School or equivalent is required.
  • Some college level education highly preferred.
  • Two years of healthcare related experience is required.
  • Experience in a doctor's office, home health agency, DME agency etc is strongly preferred.
  • Knowledge of medical terminology and basic computer skills are required. Intermediate MS Word and Excel skills preferred.
  • One year of college may be substituted for work experience

  • 8:30am to 5:00pm, includes weekends
