Represents VMG Physicians and serves as a liaison for the healthcare system, community, and state. Functions as a
team leader and lead nurse as assigned.
Consistently demonstrates service excellence. Complete at least 2 inservices per year related to quality improvement initiatites. Assist in research assignments and dissemination of results.
Provide effective and quality care management and triage via technology.
Utilize indepth interview with age appropriatequestions to obtain critical information related to presenting symptom or health concern. Continually utilize assessment skills through clinical judgement and nursing process to prioritize symptoms and/or health conditions and determine an appropriate disposition.
1. Utilize active listening and open-ended questions to obtain comprehensive patient care information.
2. Identify critical symptoms and conditions for triage and health management.
3. Utilize critical thinking skills and follow nationally accepted algorithms for triage and health coaching.
4. Demonstrate effective problem solving skills and crisis management/intervention skills.
6. Document interactions based on standards of care.Function as a patient/family advocate and promote communication and understanding between the patient and healthcareprovider to improve continuity of care.
7.Develop, implement, and evaluate a plan of care. Continually prioritize symptoms/chronic health needs and utilize criticalthinking skills to develop a collaborative plan of care and appropriate disposition.
8.Utilize motivational interviewingtechniques to obtain valuable patient data and clarify understanding of health care concerns, recommendations, andevaluate the outcomes of care planning.
9.Collaborate with patient's physician and care team regarding assessments, planof care, disposition, and outcomes.Utilizes advanced skills and assessments to develop, implement, and communicate a comprehensive plan of care which includes:
  1. Self-management interventions and education
  2. SMART goals
  3. Education of patient and family on evidence based practice related to health conditions.
  4. Identification of barriers and action planning to resolve.Maintain core competencies and stay abreast of healthcare changes that impact patient care, health system, and futuredirection. Active participant in monitoring quality improvement indicators and action plan development (including safety,quality, and patient/family experience). Assist with department level activities including:Policy and procedure development and revisions.Orient new staff to processes and procedures.Assist with medical protocol revisions and customizations.Performs as a team member by generating ideas andsupporting team's recommendations.

Minimum Requirements

  • High school plus 2 or more years of formal training/education such as Associate College Degree in Nursing.
  • 3 to 5 years Clinical nursing experience in Emergency , Pediatrics, Obstetrics, Behavioral Health or Critical Care.
  • Registered Nurse Licensure - State of North Carolina
  • 1 year of clinical experience can be substituted for clinical certification or prior triage or health coaching experience
