Vidant Medical Center is a fully accredited, 900+ bed regional referral facility encompassing a Level-1 Trauma Center and extensive inpatient and outpatient services including intensive and intermediate care, pediatrics, bariatrics, obstetrics, and gynecology, surgery, pain management and psychiatry.


  • Assists Director with the administration, development and evaluation of the Community Care Plan of Eastern Carolina at the network and district levels.
  • Oversee implementation and evaluation of Health Check and Transitional Care program initiatives in the 27 county network.
  • Hires, trains, supervises, and evaluates program staff
  • Ensures that local initiatives are congruent with the goals and objectives of the Access East and the Community Care of North Carolina (CCNC) program.
  • Utilizes various data sources to demonstrate positive outcomes related to access, quality, and cost.
  • Responsible for policies and procedures for  Network  Programs.
  • Provides leadership and support to communities in the development and implementation of Network Programs' goals and objectives. Collaborates with local leaders in all communities to plan and implement local care management initiatives.
  • Assists in data analysis to profile community health issues and gaps in service.
  • Acts as a liaison between CCNC and regional and local infrastructure. Establishes rapport with local leaders in each community.
  • Serves as a catalyst for program implementation in local communities. Provides leadership to local communities while planning and implementing program activities.
  • Ensures that local initiatives can produce meaningful pertinent outcomes.
  • Communicates program accomplishments to CCPEC Director.
  • Responsible for the development and management of CCPEC budgets for the Health Check program and regional and local CCPEC budgets.
  • Holds project coordinators accountable for adhering to budget parameters.
  • Works with CCPEC Medical Director, CCPEC Director, and Quality Nurse Specialist to analyze and communicate data to community physicians and providers.
  • Assists in the development of program evaluation plan.
  • Works with various organizations in the region and at the state level obtain necessary data.
  • Develops and implements plan for communication of provider practice data to network practices.
  • Participates in the preparation of reports and presentations for network steering committees, medical management committees, and other interest groups.
  • Assists with the development, implementation, and oversight of CCPEC special projects and initiatives.
  • Works with the Administrator and Director to determine the feasibility of special projects and initiatives to be piloted in any county or district.
  • Assists with the daily operation and implementation of special projects and grants.
  • Assists the Director in the completion of grant reports related to special pilots and initiatives.
  • Represents the CCPEC and Access East on local and statewide committees, and to organizations and granting agencies.
  • Represents the CCPEC on local and statewide committees, task forces, Boards, and workgroups and internal and external orgranizations as requested.

Minimum Requirements

  • Masters Degree required in Nursing, Public Health, Healthcare Administration or other health related field.   Nursing or MSW preferred.  

  • A minimum of 4-5 years experience working with multiple medical disciplines in disease management and chronic disease and community groups in a variety of settings is required.

  • Additional experience  may be  substituted for masters degree if experience is in community health programs involving case management and disease management.
